Event Description:

On this call, you heard how Quality Innovation Network-Quality Improvement Organizations (QIN-QIOs) and healthcare providers in low-resource practices are leveraging standardized protocols, electronic health records, and community health workers (CHWs) to better manage high blood pressure and other chronic conditions. We also explored options for CHW recognition, compensation, and professional development, and we’ll share resources from the CMS Office of Minority Health to improve the accessibility of chronic care management services.
