Everybody with Diabetes Counts
The EDC infographic provides an overview of content presented during the May 2018 QIO CEO call: initial results of a national paired analysis using three years of clinical data collected in the 11th SOW’s EDC program and highlights from two QINs that presented their individual networks’ progress to date. Mountain-Pacific and HSAG shared the results from their QIN-wide clinical data analyses and sustainability efforts, showing the impact of EDC on a range of health outcomes measured by this program.
The February 2019 QIO CEO Call featured results from the Cardiac Health task, which is summarized by the following infographic. QINs surpassed evaluation measure targets and have made a strong impact on a range of providers and health care settings through their collaborations with key partners and stakeholders, such as Million Hearts® and the Home Health Quality Improvement initiative.
Beginning in 2014, CMS invested in an initiative through the Quality Improvement Organization (QIO) Program to mobilize Primary Care practices across the nation in increasing depression and alcohol misuse screenings. The Behavioral Health infographic provides and snapshot of the purpose and results of the project.
The July 2018 Quality Innovation Network-Quality Improvement Organization (QIN-QIO) CEO Call highlighted the characteristics of Medicare Fee-For-Service beneficiaries who were high utilizers of hospital services in both 2014 and 2017. It also featured the efforts of two QIN-QIOs to mitigate high 9−1−1 or emergency department use through a fire department-run community-based program in Colorado and sickle cell disease centers in Tennessee.
