Event Description:

The call featured speakers from both the Institute for Healthcare Improvement and the Sutter Center for Integrated Care, who shared best practices in person-centered care surrounding the Always Events’ framework.

  • Always Events Getting Started Kit - This is relevant for all learners. It explains what Always events are and the IHI-developed framework and process necessary to create one within an organization.

The Blue Print for action contains detailed descriptions of staff, leadership, patient and family member roles in the creation and implementation of Always Events.

Outcome Statement:

The purpose of this session was to prepare health care quality improvement professionals to implement person-centered care strategies by exploring promising practices for the identification of what matters most to each and every patient, regardless of care provider, setting, and time. This experience helped participants demonstrate and promote person-centered care successes and identify opportunities for improvement, all of which promote advances in care that impact the Medicare beneficiaries served by the work of the QIO Program.

National LAN Follow-up Event Slides

Outcome Statement:

This session was a follow-up event to the February 2 National LAN Call: Trends in Person and Family Engagement” (PFE). It featured speakers from the Institute for Healthcare Improvement (IHI), Sutter Center for Integrated Care, and the Centers for Medicare & Medicaid Services (CMS). This call was open to QIN-QIOs, provider, partners, and beneficiaries.

The purpose of this call was to follow-up with how participants left the February 2 session in action, and to generate discussion between participants and speakers on PFE strategies and tactics.
