Top 5 Facts about Improving Adult Immunizations and Reducing Immunization Disparities

Lake Superior Quality Innovation Network (QIN), the Quality Innovation Network -Quality Improvement Organization (QIN-QIO) for Michigan, Minnesota and Wisconsin, came up with a visually impactful way to convey its immunization-related work. The organization developed a brief video that highlights five key facts about improving adult vaccination rates and reducing related disparities through the QIN-QIO’s immunization initiative. These facts include:

1.     Influenza and pneumonia are vaccine-preventable diseases. However, combined they were the eight leading cause of death in the United States in 2012.

2.     Home health agencies can attain direct assistance with improving and measuring progress on immunizations through the Home Health Quality Improvement national initiative.
