The Quality Improvement Organization (QIO) Program will be holding the next installment of its free “Sharing Knowledge, Improving Health Care“ learning series for health care providers on Tuesday, November 3, 2015 from 3-4 p.m., EST.
The virtual training session, entitled “Unleashing the Power of Data,” will feature guest presenter Stephen F. Jencks MD, MPH, who will discuss the importance of collecting and tracking data in quality improvement work; provide training on presenting data in run chart format to improve care quality and safety; and share local data integration stories, including a nursing home success story.
The session will also include a discussion about medication reconciliation and management collaboration, facilitated by Sara Butterfield and Christine Stegel of IPRO, the lead organization within the Atlantic Quality Innovation Network. They will share data, insights and best practices from participating home health agencies that reveal significant improvements in the transfer of patients’ medications during changes in care settings.
