This Eastern U.S. Quality Improvement Collaborative (EQIC) Readmission Care Partner sprint webinar covers transitions of care with community partners. The featured speakers are Gale Grunert of Lewis County General Hospital and Maria Sacco and Brenda Chapman of the Healthcare Association of New York State (HANYS). EQIC faculty will provide an overview of the Care Partner program with the goal of increasing community-based organizations’ involvement in the Care Partner model. Grunert will discuss a Care Transition Community Collaborative developed with the assistance of IPRO’s Quality Innovation Network-Quality Improvement Organization (QIN-QIO), which includes the identification and involvement of community-based organizations. The Community Collaborative helps identify patients at risk for readmissions and cooperatively mitigates those risks in coordination with the patient and care partner.
