Provider Evaluation of Communication and Preparation Tool (PRECPT)

This tool, developed by Comagine Health, is intended to improve communication between nursing home staff and providers. It allows providers to evaluate:

  • If nursing home staff identified themselves, the resident and the reason for the resident's admission to the facility without prompting 
  • The content of the call
  • If nursing home staff was prepared to answer specific questions about the patient 

The tool also includes a comment section where providers can indicate how the call could have gone better.
