Cardiac Rehabilitation Resources in Multiple Languages
Cardiac rehabilitation is a medically supervised program designed to improve cardiovascular health for those who have experienced a heart attack, heart failure, angioplasty or heart surgery. Health Quality Innovators (HQI) created a patient-facing fact sheet describing the benefits of cardiac rehabilitation, costs and how patients can get started. Translated versions of the Cardiac Rehabilitation Fact Sheet are now available in Chinese, German, Spanish and Vietnamese.
Diabetes Change Package
Qsource has created a diabetes change package specifically targeting several of our Partnerships for Community Health to help improve hemoglobin A1c rates in these communities. This toolkit includes printed resources, podcasts, and educational event recordings.
