FIT Testing Name Badge Card Template

The Fit Test Pocket Card was developed by the infection control department at Sidney Health Care. Each time staff receive a N95 they complete a FIT Testing Verification card to include their name, date, the manufacturer, model and size so when the facility goes into outbreak status, they can immediately respond by using their one-time use PPE.
