It identifies barriers and challenges to influenza, pneumonia and COVID-19 vaccination and provides interventions supported by evidence-based tools and resources to improve vaccination quality measures and rates.
The HQIN COVID-19 Vaccination Tracking Tool supports nursing homes in tracking COVID-19 vaccine administration for residents and staff. It includes boosters, additional doses and exemptions. It is an Excel workbook to use as a template, with instructions for use, for nursing homes to self-report data on their COVID-19 Vaccinations in residents and staff/healthcare personnel (HCP).
