This toolkit provides hospital quality and safety staff with examples of how each of the five PFE Best Practices can be applied as strategies to reduce all-cause harms. Offers a general model of patient and family engagement and all-cause harm reduction. PFE/Harms crosswalks are available by PFE Best Practice or by harm area.
