The purpose of this tool developed by Alliant HQIC is to provide a framework for hospital leadership and staff in the development of a health equity strategic plan that meets the CMS Hospital Inpatient Quality Reporting (IQR) Program Attestation Guidance for the Hospital Commitment to Health Equity Measure. The Leadership Engagement plan is to be reviewed and updated at least annually (per Domain 5).
