This presentation, provided by Superior Health Quality Alliance, is an active workgroup for healthcare providers, clinic managers, quality directors, NP/PAs, MDs, nurses, social workers, diabetes care and education specialists, care coordinators, case managers, community health workers/promotors/navigators and/or members of the person/patient family advisory councils. This presentation was developed to help educate and equip health professionals to share with other members, how to plan and implement effective diabetes management interventions.
