Personal Health Record (Spanish for Puerto Rico)

This Personal Health Record form modified by TMF Health Quality Institute, a Quality Innovation Network-Quality Improvement Organization, through which patients can maintain and manage their health information (and that of others for whom they are authorized) in a private, secure, and confidential environment. Patients and caregivers are encouraged to download this personal health form to take with you to your doctor visits. Remember to also bring your medications (including bottles) to your visits.
