This resource, developed by Great Plains Quality Innovation Network, was created as a “Quick Review” for Minimum Data Set staff and/or infection preventionist to access residents and their vaccination status for influenza, pneumococcal and COVID-19. The tool includes sections (tabs) for several years (2021 -2024) and also a cumulative ‘At A Glance’ tab which offers a yearly graphing feature for vaccinations given. The resource section also includes booster recommendations and updates.

This resource was developed by IPRO QIN-QIO to help nursing homes improve vaccination rates among residents, staff, friends and families by partnering with their community to host a vaccination clinic.
