The Geriatric Pain website was initially created to provide nurses and other staff working in long-term care environments and informal caregivers with access to free evidence-based pain assessment tools, pain management strategies, and resources to help identify and manage pain in older adults, including quality improvement processes focused on pain management. However, the site is applicable to other geriatric care settings and use by the interdisciplinary team.

Comagine Health’s Tailored Resources and Strategy for Teaching Motivational Interviewing Help Nursing Home Improve Resident COVID-19 Bivalent Booster Rate
An urban nursing home in the Pacific Northwest was referred by the Centers for Medicare & Medicaid Services (CMS) to Comagine Health, a CMS Quality Innovation Network-Quality Improvement Organization (QIN-QIO) for assistance with improving its resident COVID-19 bivalent booster rate. By implementing the interventions recommended by Comagine Health, the nursing home increased its resident COVID-19 bivalent booster rate by 53.9 percent from the time the facility was referred to the QIN-QIO and the last report to the National Healthcare Safety Network in January 2023.
Ready.Set.Go! Toolkit
Ready.Set.Go! is a tool developed by Convergence HQIC to help hospitals enhance the critical processes that connect pre-admission encounters to bedside rounding for a successful discharge. Patients, family caregivers, hospital staff and state partners from Convergence HQIC collaborated in a dynamic process to develop the tool and help hospitals build connections between these three touch points in the patient journey to improve outcomes and reduce readmissions.
