Community Health Workers: Addressing Social Drivers of Health

Community Health Workers (CHWs) are an important link between healthcare services and residents in the community. From facilitating healthcare access to improving the well-being of individuals, CHWs provide a service that positively impacts the lives of a community’s most vulnerable groups.  Health Quality Innovators (HQI) hosted a learning session on June 26 featuring a panel of CHWs representing a community-based organization, a Federally Qualified Health Center (FQHC) and a pharmacy. Panelists shared how they are working with individuals in the community to address various topics/conditions and community health outcomes, including unnecessary emergency department visits and hospitalizations. 

During this session, participants: 

  • Recognized the role of CHWs to support individuals in the community. 
  • Described the benefits of adding a CHW to a healthcare team. 
  • Distinguished how CHWs operate in various settings.  
  • Identified how their organization can collaborate with CHWs.
