Back to Baseline and Beyond: Community Approaches in Hypertension Control

Almost half of adults in the United States live with hypertension, and many don’t know it. High blood pressure is a leading cause and a controllable risk factor for heart disease and stroke and can contribute to worse outcomes for people who contract COVID-19. Now more than ever, paying attention to our blood pressure results and prevention efforts is essential. 

In this Back to Baseline and Beyond series provided by Superior Health Quality Alliance, health care providers and community partners will learn more hypertension control outreach methods and the tools that can be used in communities to curb the negative effects of heart disease. Participants will:

  • Learn about the latest guidelines, research, tools and resources physicians and health care teams can use to help patients reach and sustain healthy blood pressure levels
  • Receive blood pressure prevention and management resources to share with patients and families
  • Discover hypertension control outreach strategies for rural areas
