This informative webinar, provided by Superior Health Quality Alliance, discusses Cardiovascular-Kidney-Metabolic (CKM) syndrome, a new term for the complex connection between heart disease, kidney disease, type 2 diabetes and obesity. Clinic managers, quality directors, nurse practitioners and physician associates, physicians, nurses, coders, social workers, care coordinators, case managers, community health workers, promoters or navigators and/or members of patient and family advisory councils will benefit from this discussion.

Objectives:

  • Examine CKM syndrome as a new disease state that links cardiovascular disease, kidney disease, type 2 diabetes and obesity and how it relates to chronic health conditions and social determinants of health (SDOH)
  • Describe screening, symptoms, diagnosis and management of CKM syndrome
  • Examine how preventive care strategies decrease the risk of CKM
  • Understand the importance of a patient-centered approach to CKM
