Back to Basics: Chronic Conditions of Mental Health and Substance Use

This Back to Basics webinar, developed by Superior Health Quality Alliance for health care providers discusses how the chronic conditions of mental health and substance use can complicate the treatment of the more common chronic conditions of hypertension, diabetes, chronic kidney disease and tobacco misuse. When co-morbid chronic conditions are identified, increased attention to care coordination and long-term treatment approaches are needed.

• Discuss how chronic conditions can often lead to either mental health or substance use conditions.
