Back to Basics: Hypertension and Stroke Care Wisconsin Coverdell Stroke Program

The Paul Coverdell National Acute Stroke Program funds thirteen states including Michigan, Minnesota and Wisconsin to improve the delivery and quality of stroke care. This Back to Basics series provided by Superior Health Quality Alliance is intended for health care providers including clinic managers, quality directors, NP/PAs, MDs, nurses, social workers, diabetes care and education specialists, care coordinators, case managers, community health workers/promotors/navigators and/or members of the person/patient family advisory councils. 

This series will take a closer look at the Wisconsin Coverdell Stroke program which aims to support the implementation of comprehensive stroke systems for individuals at highest risk for stroke events and to improve the quality of care for stroke patients across the continuum of care. 

The following objectives will be covered during this webinar:

  • Understand the Paul Coverdell National Acute Stroke Prevention (PCNASP) program.
  • Explore how the Paul Coverdell National and Wisconsin Coverdell Stroke Program support and implement data driven quality improvement programs and activities for stroke care. 
  • Understand how the Paul Coverdell National and Wisconsin Coverdell Stroke Program are identifying disparities in stroke care.
  • Identify primary and secondary prevention activities the Paul Coverdell National and Wisconsin Coverdell Stroke Program are utilizing to educate the community.
