Circle of Safety: All-Cause Harm Prevention Model & Resource Tool

This resource, created by IPRO HQIC, was developed for hospitals to offer an integrated approach using quality and patient safety best practices to improve care and prevent adverse events that could:

• Cause patients undue harm
• Increase length of stay
• Lead to mortality
• Cause health care financial burden

It includes priority focus areas of harm for 2020-2024 including pressure injury, Central Line-Associated Blood Stream Infection, Catheter-Associated Urinary Tract Infection, C.difficile/antibiotic stewardship, sepsis, readmissions, adverse drug events and opioid stewardship/behavioral health.
