Health Equity Measure Collection Tool

In 2024, hospitals will have the opportunity to begin reporting three Centers for Medicare & Medicaid Services (CMS) health equity measures:

• One structural measure on the Hospital Commitment to Health Equity (HCHE)
• Two process measures related to screening for social drivers of health (SDOH-1 and SDOH-2).

Each tab of this worksheet includes the measure specifications, reporting dates and a simple template to help you understand the inputs you'll need to gather to report these metrics.

Your facility might pull the data for these metrics from electronic health record (EHR) reports or other sources. This template is a starting point to successfully meet CMS reporting requirements and to consider how to use these data for improvement activities within your facility.
