This reporting template was developed by Convergence Health Consulting for hospitals to use as a starting point to successfully meet reporting requirements for the Centers for Medicare & Medicaid Services (CMS) three new health equity measures:
- One structural measure on the Hospital Commitment to Health Equity (HCHE)
- Two process measures related to screening for social drivers of health (SDOH-1 and SDOH-2).
Each tab of the worksheet includes measure specifications, reporting dates and a simple template to help hospitals understand the inputs they will need to report these metrics. Hospitals might pull data for these metrics from electronic health record (EHR) reports or other sources. Hospitals may also use these data for improvement activities within their facility.
