Prevention, early detection and management of chronic kidney disease (CKD) are key to supporting patients at the primary care clinic level. Provided by Superior Health Quality Alliance, the training sessions listed below will teach health care providers techniques to work collaboratively across the health care continuum to manage CKD and proper management in the transition from CKD to end-stage renal disease (ESRD)/dialysis.

The following objectives will be covered during these sessions:

  • Support patients at the primary care clinic level through prevention, early detection, and management of CKD.
  • Work collaboratively across the health care continuum to manage CKD and proper management in the transition from CKD to ESRD/dialysis.
  • Ensure health equity is addressed in patient care.
