As part of its rapid-cycle improvement programs, the Healthcare Association of New York State and the Eastern US Quality Improvement Collaborative created new and updated assessment tools to compare current hospital operations and outcomes to evidence-based best practices.

This assessment helps hospitals practice venous thromboembolism prevention through the appropriate use of anticoagulants. Use this tool to interview unit-based staff and compare current practices with recommended evidence-based best practices.
